I just can not put into real words how passionate I am about insulin pump therapy. I mean why not? Don’t you (as a physician) always want what is closest to human physiology (when that is what is gone)? Don’t you always want the absolute best available treatment on the market for someone whose organ has stopped functioning? Don’t we do heart and kidney transplants for those whose hearts and kidneys have failed??
Do we really think that 4 injections a day can even vaguely “mimic” a very very complex physiology? Really???
Shoo ! If we do then I think we have put our brains to sleep.
My little often says to me – “how did God make the flowers mommy? “, ” how did God make the cars?” ” How did God make humans?” How did God make trees? ” I think you can see where this is going, it sure is complex.
And then we have the audacity to say that 4 injections can match eating, exercising, being, sleeping, illness, need I go on??
I am sorry but as a thinking human I beg to differ.
It is complex.
For this simple reason I shall continue to be almost militaristic in pursuit of the best case scenario for my patients and in type 1 diabetes pumps are it for the moment, (Until we get the artificial pancreas or the implantable one).
Simple really. Life is really too short to be on old-fashioned treatment when you have a disease like diabetes. Quality of life is king 🙂 Add to this a genteel tester – painless pricking and you are on a very good wicket.
Most moms will recognise the scenario of one screaming child, one literally hanging off your arm that is about to break, as you get to the till point an almighty tantrum by both when your answer to yet another sugary treat is no. Add to this a few staff issues at the office (new staff), some interesting financial challenges and some unexpected curve-balls.
Breaking your back in the pretext of the above to get all organised for yourself and two children to go away for the weekend (and I always always forget at least one crucial item like underwear). So I see all the patients booked, deal with my 4 year old who is very unwell at this point. Get everything in order (after burning the food for the weekend the previous night) and head off to face traffic standstills and hours of travelling with both children either crying or whining or both. Have a great weekend but on the last day get back to my phone with 20 missed calls!
I have never had 20 missed calls.
No-one expects disasters to happen, but sometimes they do.
Anyway, I am extremely grateful that there was not a fatality and that the worst damage was a fire in the office. We will be able to clean all of that up. Cuddoes to all involved actually, power is restored, telkom will arrive to redo all tomorrow, and life will continue as mostly normal in time to come.
Not everyone is familiar with the extra-ordinary tale of Joseph Pilates who was the founder of the Pilates exercise that has so much benefit. He was born in Germany in 1883 and was a very ill child. He came up with his own solution to the problem which was a set of exercises that he refined and are now known as pilates. Turns out my girls also quite like Katie, though not as much as Amy, but it struck me this morning whilst listening that the lyrics are just so beautifully constructed. Not only the divine eastern tones intertwined with lyrics that I like, but the difference between facts and guesses. And that fact that love is a fact and not a guess.
Call me old-fashioned and a romantic but I am just so happy my girls like quality, and quality that inspires romance. I also feel quite small, but hugely blessed.

The ever moving yet elusive target of 7 mmol/l in diabetes control seems like the holy grail for both patients and practitioners. Why is it so hard? Well, it’s like this, everything from the weather to your acute health as well as chronic health as well as meals, exercise, humidity and just about every other factor that can change affects your sugars. Not only the number but the rapidity with which is it going up or down.
Don’t feel disillusioned, be kind to yourself, especially as the patient or the care-giver : spouse or mom/dad. Always try and work out why something happened and learn from it. Sometimes though there does not seem to be a valid reason. Make peace with that episode and continue on this holy grail road. I wish I had a magic wand to “sort” this sugar spaghetti. It is down to hard work and perseverance. Sometimes the sugars do control and then all of a sudden they just seem to go “haywire”.
Stress more than anything affects them very badly so stressing about the spaghetti is not the answer.
What I can say is that we are getting closer and closer to major breakthroughs in diabetes and already in 2016 we are in a far better place than ever before. We have multiple tools in our armentarium.
The elusive 7 is the target. This target seems unattainable for many. Still aim for it. With a good team around you each day you will be closer.
